ABOUT US

Access Care Group is a Disability Support Service Provider under the National Disability Insurance Scheme (NDIS).

We specialise in providing support to all people with a disability, including:

• specialist support coordination

• information and connections to services in their communities

• home care, and

• information about support provided by the Victorian Government

​

We work under a holistic framework to be able to determine the needs of our clients physically, mentally, emotionally and socially.

Our aim is to be able to make each individual feel empowered to make their own choices and to be assisted in the aspects of their lives they determine they need help with. 

​

We also understand that an individual’s support system can be inclusive of family and close friends and engage services in a way that is fun, innovative and creative. This also includes ensuring the safety, wellbeing and ongoing provision of information to the client and their family for ongoing support at home.
